How to address the envelope
Use the address letter for letter

Add the return address
Print your return address in the top-left corner. Without it, an undeliverable letter is simply lost instead of returned to you.
Send it by certified mail
Take the letter to a post office counter and ask for certified mail: you get a postmarked receipt and a delivery record. Keep the tracking number and check it until it shows delivered.

Keep your copy of the letter, the receipt and the delivery record in one place. The delivery date is the one to quote if the account is billed again.
