How to address the envelope
Write the address as printed

Add your return address
Your name and postal address go top-left on the front (or the back flap). If the letter cannot be delivered it comes back to you, which is itself evidence of the attempt.
Send it certified
Take the letter to a post office counter and ask for certified mail: you get a postmarked receipt and a delivery record. Keep the tracking number and check it until it shows delivered.
Keep the records
