What information do you need from me?
Short answer
Your name, email and mailing address as they appear on the account, the company you want to cancel, and when the cancellation should take effect. An account or membership number is optional, but it helps the company find your account faster.
What you fill in
- The company you want to cancel, and your reason, picked from a short list.
- Your first and last name.
- Your email address, for tracking updates and the delivery proof.
- Your street address, ZIP code, city and state.
- Your account or membership number, if you have it.
- When it should end: as soon as possible, or on a date you choose.
Why the details should match the account
The company matches your letter to a customer record. A name, address or account number that does not match is the most common reason a request stalls, so use the details on the account, even if you have moved since.
Where to find your account number
It is usually on your bill, your welcome email or your member card. If you cannot find one, leave it out: your name, address and email on the account do the same job.
What you do not need
No password and no login to the company’s website. You see the full letter as you type, before you send it. Want to read one first? See our cancellation letter template.
